Sean "Diddy" Combs was taken out of FCI Fort Dix, the federal prison in South Jersey where he is serving his sentence, by ambulance early Saturday for treatment of a head injury, according to TMZ, which first reported it. Officials have not confirmed the report.

Combs, 56, was convicted in July 2025 on two counts of transportation to engage in prostitution and acquitted of sex trafficking and racketeering conspiracy. A federal judge in Manhattan sentenced him in October 2025 to 50 months in prison. He is appealing the conviction.

The report comes as his time at the low-security prison on Joint Base McGuire-Dix-Lakehurst is under a spotlight. Last month, former inmates interviewed by NBC News described special treatment for Combs, claims the Bureau of Prisons has not addressed publicly. His projected release date has also moved from Jan. 21, 2028, to March 2, 2028, according to Bureau of Prisons records.

Very little about Saturday's incident has been confirmed. TMZ's sources said an ambulance left the prison around 1 a.m. and that Combs was back before sunrise, reportedly awake and talking. TMZ said it had not confirmed whether he was taken to a hospital, and how he was hurt is unclear. The Bureau of Prisons said it does not comment on conditions of confinement, including medical status, citing "privacy, safety, and security reasons." A representative for Combs said, "We are aware of the reports concerning Sean Combs and are working to confirm the circumstances."

Neither the Bureau of Prisons nor Combs' team has released details about his condition. His appeal is pending before the Second Circuit, which heard arguments in April, and his projected release date remains subject to change.
